Prenatal care and delivery
After you have an ultrasound to confirm your pregnancy, you will have a financial conference before your next appointment to discuss your insurance’s coverage of prenatal care and delivery by Queen City Women’s Health. Payment plans will be established at that time. Auto debit is available upon request.
Gynecologic care
For all well woman exams, problem visits, ultrasounds, and followup appointments, your copay will be due at the time of the visit. If your insurance does not charge a copay, a charge will be collected based on visit type and applied to your deductible.
For in-office procedures, you will be contacted prior to the procedure and informed of your estimated cost due at time of service.
Paperwork Fees
FMLA paperwork for patients will be completed free of charge and returned within a two week period.
FMLA paperwork for family members will be completed for a fee of $25 and returned within a two week period.
Any paperwork needing completed within 72 hours will incur an additional fee of $50.
Records Request
Patient records can be sent to another provider's office free of charge. If requesting a hard copy of your records, there will be a fee of $25.
